What is the most common reason IT projects aren’t delivered on time?

From what I have seen over the years there are generally three key reason why most projects fail. First, they have agressive timeframes to begin with which may have been set from the exec wing without consultation in advance as to the viability of completing by then. Second, the project plan was constructed incorrectly due to the experience of the PM or project team. This sets the project up as though flying a plane on instruments, in the night and having broken instruments. The outcome becomes predictable. And third, the resouricng of the project team may not be able to perform at the level of productivity assumed within the project plan. Taken together, these factors above will certainly result in compounding delays and an over budget/behind schedule project, if not worse.
